Marketing I grant IC Faces Beauty Spa permission to take, use, and publish photographs and video of my treatment area(s) — before, during, and after — for marketing and promotional purposes, including the website, social media, advertising, and other media, without compensation. I understand that consenting to this is required to participate in the case study.*

Payment I understand that I will pay the full treatment price upfront. After I complete my treatment and return for my scheduled follow-up visit so that final "after" photos can be taken, I will receive either a 50% refund or a 50% credit toward a future service. I understand the 50% is issued only after the final follow-up photos are completed and received.*

Follow-up Photo Requirement — I understand I must return for at least one follow-up visit after my treatment so final "after" photos can be captured, and that my 50% refund or credit will not be issued until those photos are completed.*
